I am the Athletic Professional ("Participant") identified on this Waiver & Release participating in a professional athletic capacity as a player, coach, referee, trainer, or other athletic professional in activities informally titled, Great Gain Athletics Association Inc Events by "GGAA INC" Great Gain Athletics Association Inc Sports Mentoring Program voluntarily and at my own risk, and understand and agree to the following:

GENERAL WAIVER & RELEASE

1. Assumption of Risks: I acknowledge and represent that I am familiar with the significant risks and dangerous nature which originate from participating and otherwise engaging in athletic activities in a professional capacity, including, but not limited to basketball. I hereby acknowledge, accept and assume any and all responsibilities and liabilities pertaining to such risks, whether to myself or to others and without limitation or qualification. There have not been any express or implied representations to you on behalf of "GGAA INC" except as set forth in this Agreement.
2. Statement of Professional Fitness: I acknowledge that at the time of signing this Agreement I am in sufficient physical and mental condition to perform my professional duties in relation to basketball and other strenuous activities. I attest that my physical and mental condition will not in any way adversely affect my ability to play, oversee, coach, officiate, or otherwise professionally engage in programs, clinics, practices, services or games offered by "GGAA INC" which are rigorous and physically demanding.
3. Photo & Video Release: I hereby give permission to "GGAA INC" and its partner Great Gain Athletics Association Inc (a not-for-profit organization) to photograph, record and use photographs, voice recordings or video taken of me during the practices, games, team and community events associated with "GGAA INC" in any manner to help promote the program, programs mission and services as well as other activities and events as determined in the sole discretion of "GGAA INC" and any of its partnering organizations. Such use could include publications, media releases, announcements, electronic or otherwise, and on league websites or social media pages. I agree that I will not receive any compensation if such an image appears in any of the manners listed above or any other manner that the Great Gain Athletics Association Inc Sports Mentoring Program or partner organizations deems appropriate. I agree that such images and recordings are the property of Great Gain Athletics Association Inc Sports Mentoring Program and/or its partnering organization.
4. Professional Code of Conduct: I acknowledge and agree that as an Athletic Professional, I will conduct myself in a professional, respectful, and exemplary manner at all times during GGAA INC events. This includes demonstrating leadership and maintaining the highest standards of sportsmanship; showing respect to other professionals, participants, spectators and facilities; preventing and addressing any form of harassment, discrimination, or intimidation; enforcing all safety protocols and facility rules; and fostering a positive, supportive environment. I understand that failure to adhere to this professional code of conduct may result in immediate termination of my professional relationship with GGAA INC and potential suspension from future GGAA INC activities, without compensation.
5. Disclaimer and Waiver of Liability [ Program or Activity ]: "GGAA INC", as well as its principals, employees and affiliates, are not and will not be responsible for any injury or death arising from or related to participation in programs or activities where the risk of such injury or death has been recognized and acknowledged by the Athletic Professional by signing of this Waiver. I release "GGAA INC", including its principals, employees and affiliates, from liability for injury or death arising or resulting from any such risks or activities. "GGAA INC" and its principals, employees and affiliates are not and will not be responsible for any damage or loss of personal property suffered by me while engaged in any sponsored including travel to and from team related activities or offered programs except by gross negligence.
6. Disclaimer and Waiver of Liability [ Gym, School or location]: Any gym, school or location, as well as its principals, employees and affiliates, are not and will not be responsible for any injury or death arising from or related to participation in "GGAA INC" programs or activities where the risk of such injury or death has been recognized and acknowledged by me by signing of this Waiver. I release any gym, school or location, including its principals, employees and affiliates, from liability for injury or death arising or resulting from any such risks or activities or from any other use of premises or facilities in connection with any gym, school or location "GGAA INC" programs and activities. Any gym, school or location and its principals, employees and affiliates are not and will not be responsible for any damage or loss of personal property suffered by me while engaged in any sponsored or offered program or activity including, but not limited to any other location, except by reason of gross negligence.
7. Comprehensive Release: I do hereby, on behalf of myself and my heirs executors, administrators, assigns and legal and personal representatives, unconditionally and irrevocably release and discharge the "GGAA INC", the "GGAA INC"'s successors, assigns, owners, shareholders, directors, officers, employees, agents, representatives, attorneys, independent contractors, subsidiaries, and affiliates and each, every and all persons acting by, through, under or in concert with any of them (collectively, "Released Parties"). I further do hereby on behalf of myself and my heirs, executors, administrators, assigns and legal and personal representatives agree not to sue, or otherwise initiate legal, equitable or similar action or to otherwise file a complaint or other action for controversy resolution or otherwise seek losses, costs or expenses of any nature whatsoever, known or unknown, suspected or unsuspected, fixed or contingent, including without limitation, all personal injuries, known or unknown, and death and/or personal injuries to third parties, and injuries to property, real or personal, known or unknown, against any of the Released Parties, caused by any reason whatsoever related to the "GGAA INC" and my activities on or about the "GGAA INC"'s premises and/or facilities. This Release shall pertain to any claims which were known or unknown, anticipated or unanticipated at the time of the execution of this Agreement, including any information of any nature which, if known by me on the date of execution of this Agreement, may have materially affected my decision to execute this Agreement and specifically this Release.

By checking signing below, I acknowledge that I have read and fully understand the contents of this Waiver & Release. I am aware that this is a release of liability and a contract between myself and "GGAA INC," and I sign it of my own free will. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.

I am the Talent participant ("Participant") identified on this Waiver & Release participating in an event hosted by Great Gain Athletics Association Inc Events ("GGAA INC") Great Gain Athletics Association Inc Sports Mentoring Program voluntarily and at my own risk, and understand and agree to the following:

I hereby acknowledge, accept and assume any and all responsibilities and liabilities pertaining to such risks, whether to myself or to others and without limitation or qualification. There have not been any express or implied representations to you on behalf of "GGAA INC" except as set forth in this Agreement.
2. Statement of Participant's Good Health: I acknowledge that at the time of signing this Agreement, I am in sufficient physical and mental condition to participate in the event. I confirm that there are no health-related reasons or problems which preclude my participation in this event.
3. Photo & Video Release: I hereby give permission to "GGAA INC" and its partner Great Gain Athletics Association Inc (a not-for-profit organization) to photograph, record and use photographs, voice recordings or video taken of me by "GGAA INC" and/or associated staff or hired contractor during the event and related activities in any manner to help promote the program, programs mission and services as well as other activities and events as determined in the sole discretion of "GGAA INC" and any of its partnering organizations. Such use could include publications, media releases, announcements, electronic or otherwise, and on organization websites or social media pages. I agree that I will not receive any compensation if such an image appears in any of the manners listed above or any other manner that the Great Gain Athletics Association Inc Sports Mentoring Program or partner organizations deems appropriate. I agree that such images and recordings are the property of Great Gain Athletics Association Inc Sports Mentoring Program and/or its partnering organization.
4. Event Code of Conduct: I acknowledge and agree that as a Participant, I will conduct myself in a professional and respectful manner at all times during GGAA INC events. This includes showing respect to staff, other participants, attendees, and facilities; refraining from any form of harassment, discrimination, or inappropriate behavior; following all event protocols and facility rules; and maintaining a positive, professional environment. I understand that failure to adhere to this code of conduct may result in immediate removal from the event and potential exclusion from future GGAA INC activities, without compensation.
5. Disclaimer and Waiver of Liability [Event]: "GGAA INC", as well as its principals, employees and affiliates, are not and will not be responsible for any injury or loss arising from or related to participation in the event where such risks have been recognized and acknowledged by Participant by signing this Waiver. Participant releases "GGAA INC", including its principals, employees and affiliates, from liability for injury or loss arising or resulting from any such risks or activities. "GGAA INC" and its principals, employees and affiliates are not and will not be responsible for any damage or loss of personal property suffered by Participant during the event or related activities except by gross negligence.

Disclaimer and Waiver of Liability [Venue]: Any venue, as well as its principals, employees and affiliates, are not and will not be responsible for any injury or loss arising from or related to participation in "GGAA INC" events or activities where such risks have been recognized and acknowledged by Participant by signing this Waiver. Participant releases any venue, including its principals, employees and affiliates, from liability for injury or loss arising or resulting from any such risks or activities or from any use of premises or facilities in connection with any venue "GGAA INC" events and activities. Any venue and its principals, employees and affiliates are not and will not be responsible for any damage or loss of personal property suffered by Participant during any event or activity, except by reason of gross negligence.
